Use this form to tell us you want to make a claim due to a highway defect. Submitting this form alone does not mean you have submitted a claim. Your claim will only be formalised once you have completed and returned the detailed claim form which the Highways team will send to you.
What you will need
- You should have a reference number of the defect you suspect caused the issue reported on FixMyStreet Oxfordshire
- You will also need to provide the specific time, date and location of the incident
This form usually takes about 5 minutes to complete. Please note that after 60 minutes of inactivity the form will time out and any information you have entered will be lost.
