Oxford City Council shared ownership pre repairs claim form

Your lease includes an annual budget of £500 for maintenance inside your property. It covers:

• Supply water, gas or electricity - for example sinks, baths or pipes
• Heat your home, for example, a boiler or radiator
• Support the structure of the building e.g. windows, doors or walls
• Any administration fee or insurance excess in respect of a warranty may be claimed

Please complete this form to obtain authorisation to use this allowance for a repair that is covered by the allowance. After you complete the form, you will be told if you can go ahead with the repair and claim the allowance for this

As part of completing this form you will be asked to upload:

• Supporting evidence of the repair that is needed (e.g. photographs)
• A quote from a Trustmark registered tradesperson

You should not start to complete this form unless you have the above information ready to add to the form.

Please note:

• this form usually takes about 10 minutes to complete
• the form will time out after 1 hour of no activity and any information you have entered will be lost

If you are a shared owner at The Oval/Desborough Crescent, 40 Harefields, Plough House / Between Towns Road or Bridges Cross then please do not complete this form as your lease does not have a repairs allowance.
