You can refer yourself to the Occupational Therapist by calling the Social Services Access Team on 0845 0507666. When contacting them you must provide the following information:
The OT will assess your needs taking into account your situation and how urgent your needs are.
Depending on your needs the OT may recommend:
The OT will then make a detailed recommendation (called a Referral).
The Disabled Adaptations team carries out the work in accordance with the Referral and within financial constraints.
The Disabled Adaptations team will normally complete the work within 3 months of the Referral.
Due to the limited funding available a Referral will always be required except in the case of minor adaptations such as grab-rails.
